Shingles (herpes zoster)

Shingles, also known as herpes zoster, is a viral infection caused by the reactivation of the varicella-zoster virus (VZV), the same virus that causes chickenpox. After a person recovers from chickenpox, the virus can remain dormant in nerve cells for years, and later re-emerge as shingles. Shingles typically manifests as a painful rash with fluid-filled blisters that can appear on one side of the body.

1.     Causes: Shingles is caused by the reactivation of the varicella-zoster virus, which remains dormant in the nerve cells after a person has had chickenpox. The exact reason why the virus reactivates is not fully understood, but factors that may contribute to reactivation include aging, weakened immune system, stress, and certain medical conditions or treatments.

2.     Symptoms: The primary symptom of shingles is a painful rash that usually develops on one side of the body or face. Early signs and symptoms may include:

·       Pain, burning, or tingling sensation in the affected area

·       Itching or sensitivity to touch

·       Redness or a rash that develops into fluid-filled blisters

·       Fever

·       Headache

·       Fatigue or malaise

3.     Rash Distribution: The shingles rash typically follows the path of a specific nerve, known as a dermatome, which corresponds to the area of skin supplied by that nerve. Common sites for the rash include the trunk, chest, abdomen, or face. It usually occurs on one side of the body and does not cross the midline.

4.     Complications: Shingles can sometimes lead to complications, particularly in older adults or individuals with weakened immune systems. These complications may include:

·       Postherpetic neuralgia: Persistent pain that lasts for months or even years after the rash has healed.

·       Vision or hearing problems: If the rash affects the eyes or ears.

·       Skin infections: Bacterial infections can occur if the blisters become secondarily infected.

·       Neurological complications: Rarely, shingles can cause inflammation of the brain (encephalitis) or spinal cord (myelitis).

5.     Treatment: Early treatment can help reduce the severity and duration of shingles and prevent complications. Treatment options may include:

·       Antiviral medications: These medications, such as acyclovir, valacyclovir, or famciclovir, can help reduce the duration and severity of the rash if started within 72 hours of the rash onset.

·       Pain management: Over-the-counter pain relievers or prescription medications may be recommended to alleviate pain associated with shingles.

·       Antiviral eye drops: If the eyes are involved, antiviral eye drops may be prescribed.

·       Cool compresses: Applying cool, wet compresses to the rash can help soothe the affected area.

·       Calamine lotion: This can provide relief from itching.
